Shouldn’t I Just Lose Weight With Diet and Exercise?

Medically reviewed by our physician · Updated September 19, 2026 · Washington Trim Clinic, Bellevue, WA
The short answer

Diet and exercise always matter, and we teach both. For many people they are not enough alone, and that is about biology, not weakness. After you lose weight, your body raises hunger and burns fewer calories. Medication is a tool that makes healthy habits possible to keep.

What is true

  • When you lose weight, your body works to regain it. Hunger goes up, fullness goes down, and the calories you burn go down. In a well-known New England Journal of Medicine study, those hormone changes were still there one year after dieting.
  • GLP-1 medications such as semaglutide and tirzepatide quiet that hunger signal. Many people call it turning down the “food noise.”
  • If you take the medication and change nothing else, you can lose muscle along with fat. That is a real risk.
  • So the honest answer is not “medication instead of effort.” It is medication and effort.

What we do about it

  • Your numbers. Protein, calorie and water targets made for you: your age, height and weight, plus your scan data, such as fat percent and lean mass.
  • Your food. We review what you really eat and guide your nutrition. No crash diets. Simple habits you can keep after the medication.
  • Your movement. We recommend an exercise plan that protects muscle. The monthly 3D body scan shows if you are losing fat or muscle. The scale cannot.

Common questions

Why is it so hard to keep weight off after a diet?

After weight loss, hunger hormones rise, fullness hormones fall, and the body burns fewer calories. A 2011 New England Journal of Medicine study found these changes were still present a year later.

Will I lose muscle on semaglutide or tirzepatide?

You can, if you change nothing else. That is why we set a personal protein target, recommend an exercise plan, and measure fat and lean mass every month with a 3D body scan.

Do I still need to diet and exercise on semaglutide or tirzepatide?

Yes. The medication quiets hunger so that healthy habits are easier to keep. It does not replace them.
