Won’t I Gain It All Back When I Stop the Medication?

Medically reviewed by our physician · Updated September 19, 2026 · Washington Trim Clinic, Bellevue, WA
The short answer

If you stop suddenly with no plan, most of the weight usually comes back. That is true, and you deserve to hear it before you start. So at our Bellevue clinic we plan for the day you stop from the day you begin: protect muscle, build habits, and come off with a plan.

What is true

  • In the extension of the STEP 1 clinical trial of brand-name Wegovy (semaglutide), the medication and the lifestyle support both ended at week 68. One year later, people had regained about two-thirds of the weight they lost.
  • In the SURMOUNT-4 trial of brand-name tirzepatide, people who were switched to placebo regained about 14% of their body weight over the next year, while those who continued kept losing.
  • Obesity behaves like a long-term condition. When treatment stops, hunger signals return.
  • Some people stay on a low dose for a long time. Some come off. There is no single right answer, and the doctor decides with you.
  • Stopping all at once, with no new habits in place, is the hardest way to stop.

What we do about it

  • Protect muscle from day one. Muscle keeps your metabolism up. We measure it every month.
  • Build habits while hunger is quiet. That is the easiest time to learn them.
  • Plan the exit together. When you are ready, our physician plans with you how to come off. It does not have to be all at once.
  • Keep measuring. The monthly scan shows early if fat is returning, so the plan can change.
  • No contract. You can stop at any time.

Common questions

Do you have to take semaglutide or tirzepatide forever?

Not necessarily. Some people stay on a low dose long term and some come off. Our physician decides with you, and the program is built toward a maintenance phase.

How much weight do people regain after stopping?

In the STEP 1 trial extension of brand-name Wegovy, people regained about two-thirds of their lost weight within a year after both the drug and the support stopped. Those are trial results, not a prediction for any person.

How do I keep the weight off after stopping semaglutide or tirzepatide?

Protect muscle with protein and strength work, build eating habits while hunger is quiet, come off with a plan, and keep measuring so changes are caught early.
